Those who are eligible and need assistance with child support must make an application in person with the Child Support Division. Applications can be picked up in person at the Child Support Division during regular business hours Monday through Friday. Please bring the following information, when returning your application.
The Non-Custodial Parent’s:
• Social Security Number
• Birth Date
• Place of Birth
• A copy of your In Hospital Paternity Affidavit
• Physical Description
• Current Address
• Current Employer
• Photograph (if available)
• Father's Complete Legal Name & Current Address
• Mother's Maiden Name & Current Address
• Contact Person & Current Address
The child's:
• Social Security Number
• Date of Birth
• Place of Birth (Birth Certificate)
Also needed (if applicable):
• Divorce Decree, Support Order or Paternity documents
• Current Copy of Pay Record from Clerk's Office
• Record of cash, gifts, or support paid directly to you or child
• Copy of Court Minute Entry Entries
